29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):  a) Northwest corner - on or about 12/15/15, two employees were exposed to a fall of 13 feet while working on a fabricated frame scaffold. The scaffold was 5 feet wide and the planks are 19 inches wide and the scaffold at the working level had two planks.

29 CFR 1926.451(f)(15): Ladders were used on scaffolds to increase the working level height of employees: without satisfying the criteria of subparagraphs (i) - (iv) of this paragraph.Note:  This paragraph does not apply to large area scaffolds where the employer has satisfied the following criteria: the ladder was placed against a structure which was not a part of the scaffold and the scaffold was secured against the sideways thrust e   a) Northwest corner - on or about 12/15/15, two employees were exposed to a fall hazard. One employee was exposed a fall of 13 feet and the other employee was exposed to a fall of approximately 40 feet while working on a fabricated frame scaffold. The scaffold had an extension ladder on the platform.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level.  a) Northwest corner - on or about 12/15/15, two employees were exposed to a fall of 13 feet while working on a 13 foot high fabricated frame scaffold. No guardrails or other fall protection was used.

The employee was not trained to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.
