29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns.   A) Exterior, On lift/pallet: An employee was exposed to head injuries while working on a lift with a ledge directly above while not protected with any type of head protection, on or about October 30th, 2015.    B) Exterior, On ground: An employee was exposed to head injuries while working below another employee that was working on a lift above. The employee was not protected with any type of head protection, on or about October 30th, 2015.

29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use:   A) Pallet on lift - An employee was exposed to fall hazards of approximately 18 feet while working on a wooden pallet that was supported by a Lull lift. The employee was not provided with a personal fall arrest system or any other means of fall protection on or about October 30th, 2015.
