29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch: On or about January 5, 2016 at the Mitchell Mill Systems facility- employees were exposed to a struck by hazard from a 6'' Ball Bearing Grinder, in Bay 3 area of the shop, which had tongue guards that were improperly adjusted in excess of one-fourth inch. Abatement certification is not required for this item:

29 CFR 1910.303(g)(1)(iii): The employer did not ensure that at least one entrance of sufficient area was provided to give access to the working space about electric equipment 600 Volts, nominal, or less. On or about January 5, 2016 at the Mitchell Mill Systems facility- There was not an adequate entrance to the electrical panel box as the entrance was completely roped off with chains along with machines, wooden pallets and boxes. Abatement certification is not required for this item.
