On January 7, 2016, in the bending department, the Steelweld shear used to cut metal does not have a guard at the point of operation, exposing employees to an amputation hazard.    b. On January 7, 2016, on the plant floor, on the EVG 158, MG24, and 5P, there are a couple openings in the machine that are not adequately guarded, exposing employees to the hazard of moving parts.

On January 7, 2016, in the lube area, there are several unlabeled drums that contained chemicals such as used oil, kerosene, and mineral spirits without a label.
