29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:  a.  On or about January 8, 2016, on the north side of a 4/12 pitch roof of a new commercial building, three employees installing metal roofing was not protected from a 13 feet 2 inch fall hazard by the use of a personal fall protection system.  b.  On or about January 8, 2016, on the north side of a 2/1/2 on 12 pitch roof of a new commercial building, two employees installing metal roofing was not protected from a 13 feet 2 inch fall hazard by the use of a personal fall protection system.  c.  On or about January 8, 2016, on the south side of a 4/12 pitch roof of a new commercial building, an employee installing metal roofing was not protected from a 13 feet 2 inch fall hazard by the use of a personal fall protection system.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  a.   On or about January 8, 2015 on the north side of a new commercial construction building, the ladder used to access the top of the structure did not extend 3 feet above the upper landing surface, exposing employees to an 13 feet 2 inch fall hazard.
