29 CFR 1926.1053(b)(16): Portable ladder(s) with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identified them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired: a) Jobsite, 1094 Skyline Drive, Farmington, PA 15437: On or about September 22, 2015, a defective portable ladder approximately 14 feet in length was not immediately removed from service. The ladder was not equipped with anti slip safety shoes. The ladder was being used by employees to access steel framework between the tower and the building.
