Guardrail systems shall be installed before the scaffold is released for use by employees other than erection/dismantling crews. (a) Location - 143 Chestnut Street, Lynn, Massachusetts: On or about January 7, 2016, employees installing roof shingles from a pump jack scaffold that was 17 feet 4 inches above the ground, the pump jack was not equipped with an end rail system to protect employees from falling.
