29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person:   a) On or about 1/8/16, at the roof area of the site, Clarence, NY. Frequent and regular inspections were not conducted by a competent person that resulted in employees exposing to fall of 21 feet on a 10 in 12 roof.   NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:   a) On or about 1/8/16, at the roof area of the site, Clarence, NY. Employees, on a 10 in 12 roof performing tear-off, were not protected by fall protection systems. Employees were exposed to fall of 21 feet.   NO ABATEMENT CERTIFICATION REQUIRED
