29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) Facility- On or about December 12, 2015, an employee amputated the top of their fifth digit on their left hand while the guard was removed to adjust and then test product on the Rosback Perforator machine.

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours. (a) Facility- 155 Commerce Drive, Freedom, PA: On or about December 18, 2015, the employer did not report to the Pittsburgh OSHA Area Office that an employee amputated to top of his fifth digit, within twenty-four hours of knowing the injury occurred.
