29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated, the person certifying that the evaluation has been performed, the date(s) of the hazard assessment, and, which identifies the document as a certification of hazard assessment:  a.) Facility - On or about 1/11/16, dental hygienists and assistants have exposure to blood, other potentially infectious materials and chemicals such as disinfectants.  The employer provides personal protective equipment, such as gloves; safety glasses; gowns and surgical masks, however the employer has not verified that the required workplace hazard assessment had been performed through a written certification that identifies the workplace evaluated, the person certifying that the evaluation has been performed, the date(s) of the hazard assessment, and which identifies the document as a certification of hazard assessment.  ABATEMENT CERTIFICATION REQUIRED

29 CFR 1910.1030(h)(2)(ii): Employee training records for bloodborne pathogens were not maintained for 3 years from the date on which the training occurred:  a) Facility - On or about 2/22/16, employees, such as dental hygienists and assistants have occupational exposure to blood and OPIM.  The employer had provided training on 12/10/15, and had maintained those records, however, training records for prior years were not retained.  The employer had not retained training records for three years.  ABATEMENT CERTIFICATION REQUIRED
