Facility - On or about 1/11/16, dental hygienists and assistants have exposure to blood, other potentially infectious materials and chemicals such as disinfectants. The employer provides personal protective equipment, such as gloves; safety glasses; gowns and surgical masks, however the employer has not verified that the required workplace hazard assessment had been performed through a written certification that identifies the workplace evaluated, the person certifying that the evaluation has been performed, the date(s) of the hazard assessment, and which identifies the document as a certification of hazard assessment. ABATEMENT CERTIFICATION REQUIRED
