29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: On or about January, 07 2016, at the above addressed jobsite, an employee was demolishing/removing the roofing deck and the related roofing trusses from a commercial building flat roof top; and was working without the use of conventional fall protection (e.g. harness, guardrail or net), which resulted in the fall/injury to the employee.

29 CFR 1926.502(d)(9): Lanyards and/or vertical lifelines did not have a minimum breaking strength of 5,000 pounds (22.2 kN): On or about January, 07 2016, at the above addressed jobsite, an employee was demolishing/removing the roofing deck and the related roofing trusses from a 3-story commercial building flat roof top; wherein the Personal Fall Arrest System (PFAS) 50-foot lifeline was tied/knotted to a 2x4 and 2x6 roof truss, resulting in the reduction of the lifeline strength by 50 percent or more, (Ref: OSHA Appendix A to Subpart M), therefore exposing the employee to a fall hazard.

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level: On or about January, 07 2016, at the above addressed jobsite, two employees were demolishing/removing the roofing deck and the related roofing trusses from a 3-story commercial building flat roof top; wherein the Personal Fall Arrest Systems (PFAS) were connected to a six foot lanyard, which would have allowed the employees to have contacted the lower level (3rd floor/10'-2" ) in the event of a fall.
