On 1/11/16, at the Lula facility, when employees were using/wearing tight fitting respirators the employer did not provide a medical evaluation to that employee using the respirator.

On 1/11/16, at the Lula shop facility, mounted and marked extinguishers were not readily accessible and were blocked with tires, jacks and other stored items.

On or about 1/11/16, at the Lula work site, the employer did not ensure a 150 gallon portable oil tank was labeled as to what it contained.
