South wall stating in the middle.    f) Fire extinguisher sitting on floor in front of bathrooms.    g) Fire extinguisher sitting on floor next to main exit.

This was most recently documented throughout the shop located at 731 S. Industrial Ct., Rose Hill, KS, where all three control levers on the Clark forklift (Model GCS300MB Serial G138MB-0464-6961KOF) were missing the markings to indicate which way the operator should move the lever to avoid striking someone or something.

Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc:  The employer is failing to protect employees from struck-by hazards while engaging in production activities.  This was most recently documented at the shop located at 731 S. Industrial Ct., Rose Hill, KS, where the following pieces of equipment were not equipped with machine guarding:  a) On or about 1/12/16, an employee engaged in machining activities at the Summit lathe (Model 20X63A Serial SUM-5-168M) was exposed to struck-by hazards associated with unguarded rotating parts in that a barrier guard is not in place to keep employees out of the point of operation.    b) Lathe (Model 20X63A Serial SUM-5-168M) a safety shield is not in place.   c) Wilton drill press (Model 2404) a safety shield is not in place.
