29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  a)  At the jobsite, on the 4th floor deck, on or about 1/5/16:  Workers were performing carpentry work where a controlled access area had been established, and no leading edge work was underway, and workers were not otherwise protected from falling 30 feet to ground at the outside edge of the building.  b)  At the jobsite, on the 4th floor deck, on or about 1/5/16:  Workers were performing carpentry work where a controlled access area had been established, and no leading edge work was underway, and workers were not otherwise protected from falling 30 feet to ground at the elevator shaft.

29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around the openings:  a)  At the jobsite, on the 4th floor deck, on or about 1/5/16:  Workers were performing carpentry work where a hole cover had been unsecured to pass electrical cords through.    b)  At the jobsite, on the 4th floor deck, on or about 1/5/16:  Workers were performing carpentry work where a hole adjacent to the elevator shaft was unprotected.

Abatement certification must be submitted for this item.
