29 CFR 1926.20(b)(2): The employer did not conduct frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers: (a) Kennedy Construction, at 622 Overlook Drive, Lyons, CO 80540: On and before January 11, 2016, the exposing employer failed to conduct frequent and regular inspection of the job sites, materials, and equipment to be made by a competent person. Condition exposed employees to job site hazards such as falls due to the lack of supervision and enforcement of safety and health requirements.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems unless another provision in paragraph (b) of this section provides for an alternative fall protection measure: (a) Kennedy Construction, at 622 Overlook Drive, Lyons, CO 80540: On and before January 11, 2016, the exposing employer failed to ensure that employees performing work on a two story single family home were protected from falls by a guardrail system, safety net system, or a personal fall arrest system. Condition exposed employees to fall hazards of approximately 25 feet.
