29 CFR 1910.178(i)(1): Concentration levels of carbon monoxide gas created by powered industrial truck operations exceeded the levels specified in 1910.1000: a) On January 11, 2016, a Maintenance Mechanic was exposed to carbon monoxide at an 8 hour Time Weighted Average concentration of 108.4 ppm, which is 2.17 times the permissible exposure limit of 50 ppm. b) On January 11, 2016, a Driver was exposed to carbon monoxide at an 8 hour Time Weighted Average concentration of 188.7 ppm, which is 3.77 times the permissible exposure limit of 50 ppm. c) On January 11, 2016, a Driver was exposed to carbon monoxide at an 8 hour Time Weighted Average concentration of 134.2 ppm, which is 2.68 times the permissible exposure limit of 50 ppm. d) On January 11, 2016, a Driver was exposed to carbon monoxide at an 8 hour Time Weighted Average concentration of 205.6 ppm, which is 4.11 times the permissible exposure limit of 50 ppm. e) On January 11, 2016, a Driver was exposed to carbon monoxide at an 8 hour Time Weighted Average concentration of 168.2 ppm, which is 3.36 times the permissible exposure limit of 50 ppm.

29 CFR 1910.178(l)(1)(ii): The employer did not ensure that each operator had successfully completed the training required by paragraph (l), except as permitted by paragraph (l)(5), prior to permitting an employee to operate a powered industrial truck. On January 11, 2016, the employer did not ensure that each powered industrial truck operator had successfully completed the training required by paragraph (l) in that the mechanic operated a powered industrial truck which lead to a buildup of carbon monoxide.
