29 CFR 1926.405(g)(1)(iii)(C): Flexible cords and cables were run through doorways, windows, or similar openings: (a). Worksite located at 4003 N. Oak Trafficway, Kansas City, Missouri- Employee was exposed to a shock or electrocution hazard in that the employee had plugged in a cord and ran the cord through the doorway in which the door was smashing the cord and could potentially penetrate the outer insulation and energize the door frame.

Except as otherwise provided in paragraph (b) of this section, each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system: (a). Worksite located at 4003 N. Oak Trafficway, Kansas City, Missouri- Employee was observed working and walking near the edge of the roof and was not protected from falling by any means of fall protection. Employee was pushing snow off the roof in order to fix a leak on the roof.
