29 CFR 1910.134(k)(6): The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer: (a) On February 18, 2016, in the machine shop, the employer failed to provide training to the employees of the tool shop in the proper use, wear, and maintenance of voluntary use of a respirator.

29 CFR 1910.134(g)(1)(i)(A): Respirators with tight-fitting facepieces were worn by employees who had facial hair that came between the sealing surface of the facepiece and the face or that interfered with valve function: (a) On February 18, 2016, in the machine shop, an employee was observed with a full beard wearing a half face 3M dual filter respirator. The employee was exposed to Isocyanate hazards. The employer failed to require employees to be clean shaven while wearing a respirator
