29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift. a) 4444 Park Blvd., Montgomery AL; On or about January 12, 2016, the controlling employer CSD Interior, Inc. failed to ensure that its subcontractor Silva Flooring and tile, LLC protected its employees from fall hazards while working 25 feet above the ground from an articulated boom lift by failing to ensure the employees used the required fall protection.
