A. KDS Construction, Inc. worksite located 4330 Keystone Dr., Maumee, Ohio: On or about January 13, 2016, the employer did not ensure employees were provided with proper eye protection while operating pneumatic nail guns. Employees were exposed to potential flying object hazards while operating the pneumatic fasteners.

KDS Construction Inc. worksite located at 4330 Keystone Dr., Maumee, Ohio: On or about January 13, 2016, the employer did not ensure that the bunk of plywood was not used as a platform suspended by the rough terrain lift. An employee was standing on the plywood and elevated to the roof.

29 CFR 1926.501(b)(1) Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.       A. KDS Construction Inc. worksite located at 4330 Keystone Dr., Maumee, Ohio: On or about January 13, 2016, the employer did not ensure employees were protected from fall hazards while performing sheeting activities on the single story commercial structure. Employees were exposed to a fall distance of approximately 14.00 feet from the 8/12 pitch roof.

29 CFR 1926.1053(b)(13): The top or top step of a stepladder was used as a step.       A. KDS Construction Inc. worksite located at 4330 Keystone Dr., Maumee, Ohio: On or about January 13, 2016, the employer failed to ensure the top of a 12 ft A-framed stepladder was not use as a step by employees. The employees were using the top of the ladder to access the trusses to install sheeting. The employees were exposed to fall hazards due to improper ladder use.
