The program shall enable each employee to recognize hazards related to ladders and stairways, and shall train each employee in the procedures to be followed to minimize these hazards: On or about; December 10, 2015 Location: 20 North Middletown Road Nanuet, NY 10954 a) Employees installing metal decking as ceilings of storage units at a height of approximately 9' 9" inside a 60,000 sq ft commercial building were using the fly section of an extension ladder to access the deck. The employees were not trained properly and were unable to recognize the hazards related to improper use of ladders.
