29 CFR 1910.272(g)(1)(ii): Prior to entry, all mechanical, electrical, hydraulic, and pneumatic equipment which presented a danger to employees inside grain storage structures were not deenergized, disconnected, locked out and tagged, blocked-off, or otherwise prevented from operating by other equally effective means or methods. On or about January 7, 2016, and times prior thereto, the employer did not ensure that all mechanical, electrical, hydraulic and pneumatic equipment was lockout out and tagged, or otherwise prevented from operating by other equally effective means or methods when clearing grain from boot. Employees were exposed to the hazard of being caught in equipment.
