29 CFR 1926.25(a): During the course of construction, alteration, or repairs, debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures: (a)North side of the construction site, where a pile of debris had accumulated near a tubular welded scaffold next to the jobsite, exposing employees to the hazards of lacerations and/or puncture injuries caused by stepping on nails.

(a) NorthsideTubular Welded Scaffold, during masonry operations, an employee, who was erecting a tubular welded scaffold and the cross braces on tubular welded frame scaffolds were used as a means of access or egress. The employee was exposed to a fall hazard of approximately 35-feet above the lower level.
