29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, were not protected by protective helmets:  a.     North Wall, The Independence Press Building, 525-33 North 11th Street, Philadelphia, PA, 19123 - The employer did not provide and require the employees to wear head protection.  Employees who were working on third and fourth tiers of a multi-level fabricated frame scaffold were exposed to head injuries from falling objects.  Several concrete blocks were present on the wooden planks.  Observed On or About January 8, 2015
