On or about January 14, 2016, exit sign was not illuminated.  The exit sign was equipped with an internal illumination source that was not functioning.

On or about January 14, 2016, signs indicating the direction of exits were not utilized in the grinding area.  The exit route and to the nearest exit door was not readily apparent.

29 CFR 1910.242(b): Compressed air used for cleaning purposes was not reduced to less than 30 p.s.i. and was used without effective chip guarding and personal protective equipment:     a) Merit Tool, production area.  On or about January 14, 2016, the nozzle for the compressed air that was used to clean out the CNC machines was a metal tube with a pinched end.  A safety nozzle was not used in order to prevent pressure buildup if the nozzle is obstructed or dead-ended.

29 CFR 1910.303(b)(7): Electric equipment was not installed in a neat and workmanlike manner:     a) Merit Tool, east side of production floor.  On or about January 14, 2016, an electrical light switch style box (snap switch) was utilized as the controller to turn the Dayton bench grinder on and off.  Personnel were potentially exposed to an electrical hazard and unintended machine activation hazards because the grinder's controller box (light switch style box) was not equipped with an approved start/stop button and the switch was not protected against unintended activation.
