29 CFR 1926.304(d): Portable, power-driven circular saw(s) were not equipped with a guard above and below the base plate or shoe: (a) Front side/cut bench area, new residential construction located at 13030 Upper Meadow Lane, Milton, GA 30004: Employee was exposed to severe laceration and amputation hazards while operating a Makita (Model - 5007F) 7-1/4-inch circular saw without the lower guard in operation. A wooden shim was placed between the upper and lower guard which prevented the lower guard from retracting after a cut was made.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502: (a) Roof, new residential construction located at 13030 Upper Meadow Lane, Milton, GA 30004: Employee was exposed to falls of approximately 23-feet, 2-inches from the lower level while installing OSB plywood without a fall protection system in place. (b) Second floor over garage, new residential construction located at 13030 Upper Meadow Lane, Milton, GA 30004: Employee was exposed to falls of approximately 13-feet, 4-inches while receiving plywood from the lower level and stacking the plywood on the second floor with no fall protection system in place. J T Framing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 942203, citation number 1, item number 2 and was affirmed as a final order on 01/21/2014, with respect to a workplace located at 2356 Lahinch Court, Marietta, GA 30060.

29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous: (a) Electrical service box, new residential construction located at 13030 Upper Meadow Lane, Milton, GA 30004: The employer failed to remove a 14-gauge, 50-ft. extension cord from service as it was missing a ground pin and providing power to a Makita (Model - 5007F) 7-1/4-inch Circular Saw. The extension cord was plugged into a GFCI circuit and powering a double insulated tool.
