29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladders were not used at an angle such than the horizontal distance from the top support to the foot of the ladder way approximately one-quarter of the working length of the ladder (the distance along the ladder between the foot and the top support):    JOBSITE:  The horizontal distance from the vertical support to the base of the Werner 32-foot fiberglass extension ladder which was used by the employees to access the roof was more than one-fourth (1/4) of the working length of the ladder.

29 CFR 1926.1060(a)(1): The employer did not provide a training program for each employee using ladders as necessary, enabling each employee to recognize hazards related to ladders and the procedures to be followed to minimize these hazards.  JOBSITE:  The employees using the Werner 32-foot extension ladder to access the roof were not trained in the proper means to erect the ladder, recognize hazards, and the procedures to minimize any hazards.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels and 50-feet (15.25 m) wide or less, was not protected from falling by guardrail systems, safety nets:    JOBSITE:  The employees performing roofing work on a low slope (flat roof) were not protected from the hazard of falling 14 feet 5 inches to the ground by the use of guardrail systems, personal fall arrest systems or other equivalent means.    The Gervais Bros. Roofing, Inc. was previously cited for a violation of this Occupational Safety and Health standard {29 CFR 1926.501(b)(10)}, which was contained in OSHA Inspection Number 1049641, Citation Number 01, Item Number 001a and was affirmed as a final order on 05/14/2015, with respect to a workplace located at 46 Federal Road, Danbury, CT 06810.
