At the Preliminary Treatment Structure located at the 3510 East Laurel Road, Venice, FL 34275. (a) Employees were cutting and chipping a concrete slab when an engineering survey had not been performed. One of the employees was cutting underneath a 5,000 lb. concrete slab when the slab collapsed, fatally injuring the employee, on or about 01/14/2016.
