29 CFR 1926.303(b)(1): Grinding machines were not equipped with safety guards in conformance with the requirements of ANSI B7.1-1970, Safety Code for the Use, Care, and Protection of abrasive wheels, and paragraph (d) of this section.  a)  At the jobsite, at the front of the building, south of the Porte-cochere, on or about 1/6/16:  The Employer did not ensure a guard was used by workers who were using a hand grinder.

29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)  a)  At the jobsite, at the front of the building, North of the Porte-cochere, on or about 1/6/16:  Workers were installing a masonry façade from a scaffold where working levels were not fully planked.  Millennium Stucco was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(b)(1), which was contained in OSHA inspection number 314353749, citation number 1, item number 2b and was affirmed as a final order on 5/7/12, with respect to a workplace located at La Quinta Inn, 5394 Willow Place, Verona, NY.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level.  a)  At the jobsite, at the front of the building, North of the Porte-cochere, on or about 1/6/16:  Workers were installing a masonry façade from a scaffold where fall protection was not provided at 11 feet from ground.  b)  At the jobsite, at the front of the building, North of the Porte-cochere, on or about 1/6/16:  Workers were installing a masonry façade from a scaffold where fall protection was not provided at 17 feet from ground.  Millennium Stucco was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(g)(1)(viii), which was contained in OSHA inspection number 314353749, citation number 1, item number 2g and was affirmed as a final order on 5/7/12, with respect to a workplace located at La Quinta Inn, 5394 Willow Place, Verona, NY.
