29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: Along the west side of a 3-level residential building #16, on or about January 15, 2016, employees performing roof shingle installation were accessing the top side roof using a Werner 28' extension fiberglass ladder in which the side rails did not extend at least 3' above the landing of the roof.
