29 CFR 1926.451(g)(4): Guardrail systems installed to meet the requirements of this section did not comply with the provisions listed in subparagraphs (i) through (xv) of this paragraph.  On or about 14 January 2016, at this renovation construction site, an employee worked from a three section high scaffold platform without complete guardrail protection at a height greater than 15 feet above the ground level.
