29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:  (a) Coronado NF-Nogales RD, Water Building (Tank Pump House): On or about January 13, 2016 and at times prior thereto, an eyewash was not provided where employees chlorinate the water system with 12.5% sodium hypochlorite.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and automatic start up.  (a) Coronado NF-Nogales RD, warehouse/shop: On or about January 13, 2016 the chuck on the Sears drill press was not guarded to protect employees from exposure to the rotating chuck.   The U.S. Forest Service was previously cited for a violation of this occupational safety and health standard which was contained in OSHA inspection number 1061638, citation number 1, item number 5 and was affirmed as a final order on November 20, 2015, with respect to a workplace located at Prescott NF, Verde RD, Camp Verde, AZ.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:  (a) Coronado NF-Nogales RD, warehouse/shop: On or about January 13, 2016, the distance between the periphery of the abrasive wheel and the tongue guard on the Delta Shop Master bench grinder was approximately 1 inch.   The U.S. Forest Service was previously cited for a violation of this occupational safety and health standard which was contained in OSHA inspection number 982195, citation number 2, item number 4 and was affirmed as a final order on December 19, 2014, with respect to a workplace located at Angeles NF, Valyermo Hot Shots, Valyermo, CA.

The U.S. Forest Service was previously cited for a violation of this occupational safety and health standard which was contained in OSHA inspection number 1061629, citation number 1, item number 4 and was affirmed as a final order on 11/20/2015, with respect to a workplace located at Willow Administration Site, 4000 Katahn Dr., Prescott, AZ.

29 CFR 1910.305(b)(2)(i): Each outlet box in completed installations did not have a cover, faceplate, or fixture canopy:  (a) Coronado NF-Nogales RD, Recreation Supervisor's Office: On or about January 13, 2016, an outlet receptacle next to the work table was missing a faceplate and as a result the receptacle's live conductors were exposed.   The U.S. Forest Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 982175, citation number 2, item number 5 and was affirmed as a final order on 12/19/2014, with respect to a workplace located at Chileo Fire Station 15-16 Star Route, La Canada, CA.
