29 CFR 1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached nor was it designed, installed and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person.  a)     Student Officer's Quarters, Quantico Marine Corp Base: On or about 01/12/2016, two employees were attached to a Guardian Temper Anchor Part # 00445 anchored into a metal roof approximately 24 feet from the ground that was not rigged support at least 5000 pounds per man, as a result employees were exposed to a possible fall.  No abatement certification or documentation required.

29 CFR 1926.503(a)(2)(iii): The employer did not assure that each employee has been trained, as necessary, by a competent person in the correct use and operation of personal fall arrest equipment.  a)   Student Officer's Quarters, Quantico Marine Corp Base: On or about 01/12/2016, employees were not properly trained in the use of the Guardian Temper anchor part 00445 in that employees thought the worker weight capacity range written on the anchor was the combined weight of employees. The manufacturer instruction specified one man per anchor.      Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
