(a) The plywood cover placed over a hole on the grit level was not secured to the floor, exposing employees to a fall hazard, observed on or about 01/14/2016. (b) Pipe holes in the grit level floor were not covered, exposing employees to a trip hazard, observed on or about 01/14/2016.
