29 CFR 1910.22(b)(1): Aisles and passageways were not kept clear and in good repair, with no obstruction across or in aisles that could create a hazard. Inside the main mail processing room area, employee(s) were exposed to slip, trip and fall hazards observed in two (2) instances: 1. Manual Flats, 1st Floor (across from 633 Cage) - A damaged tile measuring approximately 12 inches x 24 inches, contained two inclusions (each measured approximately 5 inches x 3 inches x 1/2 inch) that resulted in a tripping accident where an employee was injured. 2. City Manual Flats, 1st Floor (north of J4) - An uneven surface created by a missing tile measuring approximately 6 inches X 6 inches X 1/2 inch resulted in a tripping incident.
