On January 16, 2016 and at times prior thereto, at the Peroxychem facility in Pasadena, TX, the contract employer did not assure that each of its employees were trained in the work practices necessary to safely vacuum hydrogen peroxide and work solution (WS) mixture from T-4031 and then transfer the mixture to one of the SM tanks.
