29 CFR 1926.350(f)(4): A hose which has been subjected to flashback, or which shows evidence of severe wear or damage, was not tested to twice the normal pressure to which it was subjected, but in no case less than 300 p.s. i. :  a)    Fort Myer Commissary Refrigeration Upgrade: On or about 01/13/2016, oxygen and acetylene hoses used in soldering copper pipes were not tested to at least 300 psi after repairs were made as a result employees were exposed to possible burns.        No abatement certification or documentation required.

29 CFR 1926.501(b)(3): Each employee in a hoist area was not protected from falling 6 feet (1.8m) or more to lower levels by guardrail systems or personal fall arrest systems:  a)    Fort Myer Commissary Refrigeration Upgrade: On or about 01/13/2016, employees standing in the hoist area after loading were not provided with fall protection as a result employees were exposed to possible 20' fall.  No abatement certification or documentation required.
