29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s): (a) Inside the Mail Processing Plant: A Toyota Electric Forklift Truck, Model 5FBE15, Serial No. 33375, with the below defects was not taken out of service until restored to safe operating conditions: 1. A broken left headlight mounting metal bracket was secured with a plastic tie wraps. 2. Rear turn signal lights did not work. 3. Rear back up left light did not work. 4. Rear stop lights did not work.
