NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. 29 CFR 1926.502(d)(11): Lifelines were not protected against being cut or abraded: a- Roof/Elevator Shaft - 535 Flushing Avenue, Brooklyn NY: On or about 01/12/16 an employee working from the top buck of a tubular welded framed scaffold was exposed to fall hazards, in excess of 19 feet to the ground surface below, when one end of their full body harness's lifeline was attached/wrapped around the unprotected edges of a steel eyebeam. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
