Autoneum North America, Inc. at the worksite located at 4131 Spartan Dr. in Oregon, Ohio: On or about January 19, 2016, employees operated a powered industrial vehicle(s) to lift and maneuver materials such as, but not limited to, insulator and carpet part racks. The employer did not assure that employees successfully completed a forklift operator training program containing elements in 29 CFR 1910.178(l) before being permitted to operate the forklift.

29 CFR 1910.178(n)(6): Industrial truck drivers were not required to look in the direction of, and keep a clear view of the path of travel: a. Autoneum North America, Inc. at the worksite located at 4131 Spartan Dr. in Oregon, Ohio: On or about January 19, 2016, the employer failed to ensure that forklift operators had a clear view of the path of travel while transporting insulator racks along the west wall, exposing workers/pedestrian to struck-by injuries. Forklift operators travel forward with the load elevated. b. Autoneum North America, Inc. at the worksite located at 4131 Spartan Dr. in Oregon, Ohio: On or about January 19, 2016, the employer failed to ensure that forklift operators had a clear view of the path of travel while pushing rows of insulator racks forward in the staging area. The operator's forward view of travel was blocked while pushing stacks of racks which were up to six racks high.

Autoneum North America, Inc. at the worksite located at 4131 Spartan Dr. in Oregon, Ohio: On or about January 19, 2016, the employer failed to ensure that forklift operators, transporting loads of insulator and/or carpet racks along the west wall, traveled with loads lowered. Operators traveled with the load elevated between approximately two and a half feet and up to six feet above the road surface, exposing employees to struck-by injuries.

29 CFR 1910.178(n)(8): Under all travel conditions the truck was not operated at a speed that would permit it to be brought to a stop in a safe manner. a. Autoneum North America, Inc. at the worksite located at 4131 Spartan Dr. in Oregon, Ohio: On or about January 19, 2016, employees were exposed to struck-by hazards in that a forklift(s) used in sequencing area in front of the shipping office did not travel at a speed which brought the forklift to a stop in a safe manner. A forklift which suddenly stopped in front of an office area resulted in the load to become removed from the lift's forks.
