29 CFR 1910.28(a)(4): Scaffolds and their components was not capable of supporting without failure at least four times the maximum intended load. On or about January 19, 2016, at the East Citgo facility, employee's were exposed to fall hazards when climbing a scaffold ladder which was not capable of supporting at least four times the maximum intended load.
