29 CFR 1926.451(a)(1): Scaffolds and scaffold components were not capable of supporting, without failure, its own weight and at least four times the maximum intended load applied or transmitted to it: (a)8207 2nd Ave., Stone Harbor, NJ: On or about January 14, 2016, two J Hughes Builders LLC employees were working from a scaffold pic that was supported by job-built 2x4 structural lumber outriggers. The scaffold outriggers were not capable of supporting the load and failed, allowing the workers to fall approx. 19 feet to the ground below.

(a)8207 2nd Ave., Stone Harbor, NJ: On or about January 14, 2016, two J Hughes Builders LLC employees were working from a scaffold pic that was supported by job-built 2x4 structural lumber outriggers. The scaffold did not have any guardrails and the employees were not using any form personal fall protection.
