29 CFR 1910.36(d)(1): Employees must be able to open an exit route door from the inside at all times without keys, tools, or special knowledge. A device such as a panic bar that locks only from the outside was not provided.  Location: 66 Okner Parkway, Livingston, NJ.  a) In the event of an emergency some employees may require special knowledge on how to operate the door in its current condition. There was a door knob with a lock at the emergency discharge door; on or about 12/11/15.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.  Location: 66 Okner Parkway, Livingston, NJ.  On or about 12/1/2015:  a) The manual string tie machine did not have a guard for the pulleys/belts thereby exposing employees to moving parts.  b) The WR-18 operator machine did not have a guard for various moving parts thereby exposing employees to the moving parts.  c) The Kluge EHD Series 14 x 22 (Kluge) press machines' safety gates did not have an interlock/latching system.  The operators were exposed to moving parts when they opened the safety gate in order to correct stock/envelope misfeeds.  d) All the old printing machines being utilized by employees at the facility did not have a  guard for various moving parts thereby exposing employees to the moving parts.

29 CFR 1910.303(b)(2): Installation and use. Listed or labeled equipment shall be installed and used in accordance with any instructions included in the listing or labeling.  Location: 66 Okner Parkway, Livingston, NJ.   a) Employer utilized light duty relocatable power taps (RPT) and temporary extension cords as a permanent connection to power various machines such as, but not limited to, the manual string tie and Fillpack II Rampak machines.  The extension cords and RPTs' were not used as specified as employer used it to capacity daily thereby increasing overheating and thus creating a potential fire hazard at the facility; on or about 12/11/15.
