29 CFR 1910.147(c)(6)(i): The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed. The employer conducted periodic inspection on the Prime UV, UV#2 on the Sanden Printing Machine on July 22, 2015 by an authorized employee was inadequate in that the inspection did not ensure that all hazardous energy sources were isolated and energy isolating devices affixed prior to the start of the servicing and maintenance activity.

29 CFR 1910.147(c)(7)(i)(A): Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control. The training provided to authorized employees was inadequate in that lockout hardware in conjunction with a lock for a pneumatic ball valve was unavailable to authorized employees to affix onto a pneumatic ball value of the Sanden Press Machine and other machines with pneumatic energy sources.

29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: On or about January 12, 2016, in the Production Area, in the rear of the Sanden Press Machine, Offset #7, Press Operators were not required to: (1) know the type and magnitude of the energy, the hazards of the energy to be controlled, and the method or means to control the energy (2) turn off and shut down the machine (3) physical locate and operate energy isolating devices in such a manner as to isolate the machine from the energy source (4) affix lockout device to each energy isolating device (5) prior to starting work on machines that have been locked out, verify that isolation and deenergization of the machine have been accomplished.

29 CFR 1910.147(f)(1)(ii): Remove employees from the machine or equipment area in accordance with paragraph (e)(2) of this section; On or about January 12, 2016 at around 1:15 pm in the Production Area at the Sanden Press Machine, a Press Operator was exposed to an amputation hazard when he did not remove a part of his right ring finger from the gears in Offset #7 while another employee turned the switch out and in to see if the roller would turn.

29 CFR 1910.303(b)(8)(i): Electric equipment shall be firmly secured to the surface on which it is mounted. Note to paragraph (b)(8)(i) of this section: Wooden plugs driven into holes in masonry, concrete, plaster, or similar materials are not considered secure means of fastening electric equipment. In the rear of the Sanden Press Machine, Offset #7, an employee was exposed to an electric shock hazard when a 110 Volt AC two receptacle outlet box was free hanging and not mounted.
