29 CFR 1915.73(e): When employees were working near the unguarded edges of decks of vessels afloat, they were not protected by personal flotation devices meeting the requirements of 29 CFR 1915.158  (a) Barge - On or about 1/20/16, employees were exposed to water hazards when working from the floating barges without the proper personal flotation devices. 29 CFR 1915.73(d): When employees were exposed to unguarded edges of decks, platforms, flats, or similar flat surfaces, more than 5 feet above a solid surface, the edges were not guarded by adequate guardrails :  a) Barge - On or about 1/20/16, employees were exposed to fall hazards  when the walkway-platform was not protected by guardrails.  b) Platform - On or about 1/20/16, employees were exposed to fall hazards when the  platform used to access the barges was not equipped with guardrails.   In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).  The R and D Associates, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1915.73(d), which was contained in OSHA inspection number 301511549, citation number 1, item number 2b and was affirmed as a final order on 3/3/2011with respect to a workplace located at 100 10th Street, Catlettsburg, KY 41102.
