29 CFR 1904.39(a)(1): The employer did not report within 8-hours the death of an employee resulting from a work-related incident to the Occupational Safety and Health Administration (OSHA), U.S. Department of Labor: On or about January 12, 2016, on Hwy 181, near the Buddy Ganem exit, an employee performing weed-eating operations was killed when struck-by an automobile and the employer did not report the fatality to OSHA.
