29 CFR l 926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652( c): a) Jobsite, 150 Old Bury Drive, Wilmington, DE 19808: On or about January 20, 2016, two employees were working in a trench that was up to approximately 15 feet deep. Tri State Grouting, Inc. was previously cited for a violation of this Occupational Safety and Health Standard, 29 CFR l 926.652(a)(1), which was contained in OSHA inspection number 982903, citation number 1, item number 1a and was affirmed as a final order on 8/12/2014 with respect to a workplace located at 3717 Grant Street, Castle Shannon, PA 15234.
