29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes: (a) South Roof: On or about January 18, 2016 and at times prior, as the controlling employer, Oswood Construction Company did not ensure that its subcontractors Bachall Construction protected their employees in that employees were exposed to a fall of approximately 18 feet to the floor below while walking and working in and around an unguarded hole. (b) North Roof: On or about January 18, 2016 and at times prior, as the controlling employer, Oswood Construction Company did not ensure that its subcontractors Bachall Construction protected their employees in that employees were exposed to a fall of approximately 18 feet to the floor below while walking and working in and around an unguarded hole.
