29 CFR 1926.451(b)(1): Platform(s) on all working levels of scaffolds, were not fully planked or decked between the front uprights and guardrail supports such that the space between adjacent units, and the space between the platform and the uprights.    a)  At the jobsite, adjacent with the 14th floor, on or about 1/20/16:  An employee was working on a scaffold platform approximately 140 feet from ground where that platform was not fully planked.    Park Side Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(b)(1), which was contained in OSHA inspection number 1009139, citation number 1, item number 1, and was affirmed as a final order on 5/19/15, with respect to a workplace located at 9 Orchard Street, New York, NY.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used:    a)  At the jobsite, adjacent with the 14th floor, on or about 1/20/16:  An employee was working on a scaffold platform approximately 140 feet from ground and egressed that platform by climbing onto a needle beam that was approximately 3.5 feet from the platform.      Park Side Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(e)(1), which was contained in OSHA inspection number 1009139, citation number 1, item number 2, and was affirmed as a final order on 5/19/15, with respect to a workplace located at 9 Orchard Street, New York, NY.
