29 CFR 1926.1053(a)(7): Ladders not specifically designed for such use were tied or fastened together to provide longer sections: (a) North side, new residential construction located at 3017 Devoncroft Street, Smyrna, GA 30080: Employee was exposed to falls of approximately 38-feet while using a ladder which was tied together with an orange strap to make the ladder longer. The ladder's fly section was extended past the ladder locks which did not allow the locks to be fully engaged with the rungs of the base section of the extension ladder.

The employee using this ladder to access the roof was exposed to falls of approximately 38-feet to the lower level.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would train each employee in the procedures to be followed to minimize hazards related to ladders and stairways: (a) New residential construction located at 3017 Devoncroft Street, Smyrna, GA 30085: Employees using an extension ladder failed to use the ladder at a proper angle, modified the ladder by tying the two sections together with a strap, and failed to extend the ladder 3-feet past the roof landing which exposed the user to fall hazards. Employees were not trained to recognize that these hazards could cause injuries or death.
